The West London NHS Trust rough sleepers’ service plays a key part in responding to the needs of rough sleepers who also have mental health and physical health difficulties. The service works across Hammersmith and Fulham, Ealing and Hounslow with a range of partners, including St Mungo’s outreach service and local authority homelessness departments.
The team works flexible hours including late-night and early morning shifts in order to support those who need the service. It assesses mental and physical health needs, and helps enable rough sleepers to connect with relevant health and appropriate accommodation services.
The team also provides some in-reach support to those rough sleepers who are in temporary accommodation such as hostels or hotels.
