NHS Ealing Community Partners’ Dementia Services offer people diagnosed with dementia and their carers support from the point at which they’re first assessed.
Working with Dementia Concern, the service provides advice, information and practical support to help people with dementia to live as independently as possible. Its dementia link workers provide support and advice to families. This often helps people with dementia stay in their own homes. Its monitoring and support service helps people with dementia who live alone.
The service provides a range of support services for people diagnosed with dementia, which also provide respite for carers. Care is provided in sites across the London Borough of Ealing.
The service works closely with the West London NHS Trust’s Older People’s Mental Health Services (OPMHS), who support patients with a range of mental health conditions, including dementia.
